Effects of Intravenous Erythromycin on Antroduodenal Motility in Humans: Non-Invasive Observations with Real-Time Ultrasound

靜脈注射紅黴素對人類胃十二指腸蠕動之作用:非侵犯性超音波之觀察

本研究之目的是利用超音波檢查去評估紅黴素如何影響胃十二指腸之蠕動而達到加速胃排空之結果。從1994年一月到十月,共有15個自願者(10個男性及5個女性,平均55.2歲)進入本研究。在空腹一個晚上後,隔日早上喝了500cc含麥粉粒之液態柳橙汁後立刻開始接受超音波檢查。我們觀察胃竇之收縮經幽門之流動及胃-幽門-十二指腸協同性蠕動之情形。首先點滴靜脈連續滴注生理鹽水當作安慰劑,然後在第10分鐘後開始給予靜脈注射紅黴素(12.5毫克/分鐘)10分鐘。同時為了評估此實驗方法之可信度,我們比較了12個健康自願者之前後10分鐘胃十二指腸蠕動之差別。結果顯示:一、比較12個健康自願者之前後10分鐘胃十二指腸蠕動,並沒有統計學上之差別。二、與安慰劑比較,紅黴素不影響胃之收縮週期和經幽門逆流之次數及時間。但是增加了胃-幽門-十二指腸之協同性蠕動而達到其加速胃排空之結果。

Erythromycin has been shown to act on motilin receptors on gastrointestinal smooth muscle in vitro and to accelerate gastric emptying in normal subjects as well as in patients with diabetic mellitus. To evaluate the motor pattern that accounts for this accelerated emptying, the effects of 12.5 mg/min erythromycin vs. placebo on postprandial motility of the antroduodenum was examined with real-time ultrasound in 15 normal subjects. During 10 minutes of observation, erythromycin significantly increased forward transpyloric flow epsode (1.04±0.19 vs. 0.37±0.41; p<0.05), forward transpyloric flow duration (5.79<4.49 vs. 0.21±0.17; p<0.05). However, no significant differences were found for gastric peristaltic cycle (22.62±3.06 vs. 0.18±0.29; p>0.05), retranspyloric flow episode (1.24±0.30 vs. 1.38±0.58 seconds; p.0.05). We conclude that erythromycin increases episode and duration of forward transpyloric flow, and improves antro-pyloro-duodenal coordination, which may play a role in accelerating gastric emptying.
