「執業導師」課程可行性之初探

The Practicability of Preceptorship in the Curriculum of Medical Education

摘要


傳統的醫學教育重視個人器官系統疾病處理,普遍缺乏社區醫療照顧的觀念及能力。「執業導師」課程是指醫學生在不同醫學教育階段中,加入在基層執業場所的學習經驗,這是美國醫學教育的一大改革。本文選取現任職於教學單位或基層執業的家庭醫學科醫師為研究對象,以郵寄問卷方法,調查對現行醫學生之家庭醫學課程和「執業導師」課程的意見,藉此評估執業導師課程於台灣地區的可行性及預期障礙,希望提供相關單位於醫學課程規劃上的參考依據。235份(回應率40.8%)回應問卷分析結果,發現多數回應者對現行的家庭醫學課程不甚滿意,但大多同意基層醫師可以幫助醫學生之醫療訓練,並認為應於基礎課程中及早加入。故建議家庭醫學教學活動的規劃者,確實檢討現行之課程,並考慮「執業導師」課程在台灣地區的適用性,注意實前師資之培訓及實施後之評估工作。

Physicians who have graduated from traditional medical education curriculum focus on techological and organ system training, and are deficient in modern community health care concepts and skills. A preceptorship course integrates at various medical educational periods is one of the important goals at education reform in America. In this study, we attempt to ascertain the opinions of family physicians about the current family medicine curriculum and the practicability of preceptorship in Taiwan. The result can be a reference for future medical curriculum design. Two hundreds and thirty-five(40.8%) questionnaires were collected and analyzed. They showed most respondents were dissatisfied with the current curriculum of family medicine, and most agree that primary care physicians involvement would be helpful in producing efficient and well-qualified physicians. They suggested that preceptorship should best be first integrated in a basic knowledge course. We suggest that faculties who are responsible for curriculum design should evaluate the current curriculum and the appropriateness of preceptorship in Taiwan. Prior to the reform, faculty development and evaluation are the essential and crucial tasks.
