Intrasellar Meningiomas
蝶鞍內腦膜瘤
摘要
位於蝶鞍內的腦膜瘤十分罕見，文獻上已知的不超過二十例，我們提出兩病例討論其病史、診斷、手術治療及預後。 由於其臨床徵象，這兩個病例術前的診斷為巨大型腦垂腺瘤，生化檢查顯示除了促乳素升高外，其餘血清中腦垂腺激素濃度偏底，這可能是由於腫瘤壓迫到腦垂莖部所致。而在術前要鑑別診斷，腦垂腺瘤或蝶鞍內腦膜瘤十分困難。 兩病例均接受經蝶鞍的腫瘤切除術移除腫瘤而獲致良好的結果。

Two cases of intrasellar meningiomas with preoperative diagnosis of pituitary ma-crodenomas are presented. The preoperative hormonal studies revealed decreased serum hormonal levels with a slightly elevated prolactin level in both cases, which may have been due to the compression of the hypophyseal stalk. It is difficult to differentiate in-trasellar meningiomas from pituitary tumors. Correct preoperative diagnosis depends on clinical picture, neuroimaging and endocrine studies. Surgically, the transsphenoidal ap-proach provides a relatively safe route to the intrasellar meningiomas and satisfactory results.
