摘要

本文乃探討一位病患頭部遭受重擊後，因腦創傷伴隨的神經損傷，導致肢體活動，語言表達及心理層面受損，無法與人互動或溝通，承受身、心的痛苦的疾病過程，護理過程中以Gordon十一項護理評估，確立其護理問題有：身體活動功能障礙、言辭溝通障礙、自我照顧能力缺失及身體心像紊亂，針對生理、心理、社會及靈性等問題，給予整體性照護，鼓勵個案與家屬學習自我照顧及心理調適，並走出創傷陰霾之護理經驗。

This paper will show the progress of the disease for a patient who suffered from a head injury with neurological deficit, and compromised his motor skill with speaking and communication difficulties. In the hospital his condition was evaluated by the Gordon 11 nursing assessment. The assessment showed four problems. Defect of physical activity, Defect of self care ability, Communication problems, and Physiological function disturbance. We should make total care of the patient physiologically, mentally and familially.
