以病患需求為導向之精神病患出院準備服務概念
Need-led Psychiatric Discharge Nursing Care Concept
摘要
本國當今的精神醫療照護體系，仍以急性住院照護爲服務重點，對於出院後的「後績照顏」刻尚未有妥善規割。本文提出「以病患需求爲導向之精神病患出院準備服務概念」並舉實例探討，期作爲精神醬療照護中出院準備服務或社區照護之參考。

In Taiwan, the most important service of the psychiatric care system is still focused on acute care more than aftercare. A need-led psychiatric discharge care concept was introduced to guide the discharge care and the recommendations for future implementation and application were also discussed in this article. More attention should be placed on psychiatric discharge care in order to increase psychiatric outpatients' community adaptation, and the need-led psychiatric discharge care concept could be applied in the discharge transitional process.
