Increasing the Thoroughness of Nursing Instruction during Pre-examinations in the Urological Department

摘要

本專案主旨是提昇泌尿外科病房執行各項檢查前護理指導之完整率。經由查檢、觀察護理人員對住院病人之檢查前護理指導執行率為64%，完整率為56%。分析各項檢查前護理指導不完整的原因為：檢查排程時間不確定，導致檢查前準備的時間不足、各類檢查護理指導單張不足、專業知識不足等。綜合以上護理指導完整率缺失，自2004年3-12月進行本專案改善，包括提早給予病人檢查前護理指導，及藉由持續監測護理指導執行率，以提昇護理指導完整性。經由本專案改善後，病人檢查護理指導執行率達94%，完整率為98%；病人滿意度由執行前89%提昇為100%。值得在臨床上繼續推展。

The purpose of this project was to increase the thoroughness of nursing instruction during pre-examinations in the Urological Department. Observation of nurses checking admitted patients during the examination showed that the nursing instruction performance rate was 64% and the thoroughness rate was 56%. By analyzing the lack of thoroughness, the reasons were as follows: 1. Uncertainty about the schedule, without enough preparation before the examination. 2. Lack of nursing instruction notes for examinations. 3. Insufficiency professional knowledge.
From March to December 2004, we started this improvement project by giving nursing instruction earlier, and continuously monitoring the nursing instruction performance rate to increase the thoroughness of nursing instruction.
After the improvement of this project, the performance rate was raised to 94% and the thoroughness rate was raised to 98%. The satisfaction rate of patients was raised from 89% to 100%. To conclude, this project is worth distributing system-wide.
