喪親照顧者對安寧緩和醫療病房善終服務滿意度的探討

本研究目的主要是想瞭解喪親照顧者對安寧緩和醫療病房善終服務之滿意度及其相關影響因素，收案方式以立意取樣法於中部某教學醫院安寧緩和醫療病房，針對瀕死出院或在院宣佈死亡病人之喪親照顧者65位，以郵寄問卷方式收集資料，並以SPSS10.0版進行描述性統計及Pearson積差相關資料分析。共收得有效問卷52份（回收率80%），研究結果顯示：喪親照顧者認為病人對善終之體認分數，平均為6.96±1.85分（滿分11分）。63.5%喪親照顧者認為病人對自己不久於人世的事實為部份接受；44.2%身後事為妥善交代安排；63.5%病人辭世時間為剛剛好；51.9%病人去世前三天為些許不適。善終服務滿意度平均為4.0±.8分（5分量表），其中病人自主能力得到尊重平均得分最高（4.52±.64分）；協助家屬依病人宗教信仰作死亡前的準備平均得分最低（3.42±1.53分）。本研究結果可作為促進安寧緩和醫療病房善終服務品質之參考。

This study aiming at understanding the perception of ”good death” of terminal cancer patients by their bereaved caregivers and their level of satisfaction with the care given. Sixty-five bereaved caregivers were purposely selected. The patients they took care of either died in or were discharged in impending-death status from a palliative care unit. Mailing questionnaires were given and the data were analyzed by descriptive statistics and person correlation from SPSS (10.0 edition). Totally, 52 caregivers responded (responding rate: 80%). The results showed: The average ”index of good death” was 6.69 on an 11-point scale, 63.5% of the care-givers perceived that the deceased partially accepted at least the reality of facing the end of life, 63.5% of the caregivers perceived the timing of death as appropriate, 44.2% of the deceased had already made arrangements for matters following their death, and 51.9% of the deceased had some physical distress in the last three days of their lives. The average level of satisfaction of the care given was 4.0 (SD=.80) on a 5-point scale. The item ”the patient's autonomy to be expected” received the highest score (4.52), while the item ”to assist the family to prepare pre-dying ritual according to their religious belief” received the lowest score (3.42). The result of this study could be used as a reference for making improvements in the quality of care provided in the palliative care unit.
