一位行會陰切除術婦女性生活調適之護理經驗
Nursing Experience of a Female Patient's Sexual Adjustment with Perineal Resection
摘要
本文探討一位罹患會陰癌第三期婦女，接受根除性會陰切除手術而面臨的生理及心理調適問題。筆者於2008年10月12日至2008年10月23日護理期間，透過觀察、會談、身體評估、電話訪談等方式收集主、客觀資料。運用Gordon十一項功能性健康型態評估個案，發現主要的健康問題有：焦慮、知識不足、疼痛、身體心像紊亂、性生活型態改變等五項。藉由擬定個別性護理計劃，維持個案身、心之完整；運用同理心及真誠的態度，與個案及家屬建立良好的護病關係，建立正確性觀念；鼓勵婦女重建自我的信心及界定自己，順利渡過手術所帶來的性生活改變，進而調適及維持夫妻雙方親密關係。期望提供臨床護理人員照護參考，鼓勵婦女勇於重視及面對性生活改變，進而運用因應策略提升婦女性生活滿足感。

The purpose of this article was to describe the physical and psychological problems of a woman suffering from vaginal cancer and undergoing radical perineal resection. The period of nursing care was from October 12 to 23 in 2008. Data was collected through observations, interviews, physical examinations and telephone communications. Using Gordon's element-item health assessment scale, the author found five health problems: anxiety, lack of knowledge, pain, body image disturbance, and sexual lifestyle changes. With individualized care plans, empathy and sincere attitude, the author encouraged the case to rebuild her own self-confidence, to adapt to changes in her sexual life, and to maintain a close relationship with her husband. The author hoped that the nursing experience gained from this case could be valuable for clinical workers involved in such cases and also be able to encourage women to face changes in sexual life as well as to take on coping strategies to increase their sexual satisfaction in the future.
