This article discusses the clinical professional care in which nurses need to provide to patients with acute ischemic stroke involving the use of rt-PA. Among the acute stroke patients in Taiwan, 70%suffer from ischemic stroke. In the meantime, the only therapy for them is rt-PA. However, it should be noted that: (1) rt-PA must be used within three hours after onset of stroke, and (2) 7% of the patients treated with rt-PA may suffer from complications such as cerebral hemorrhage, and even deaths. Therefore, nurses providing first line of care patients' need not only understand the conditions of rt-PA therapy, including the conditions to accept or to exclude, but also be equipped with pharmaceutical knowledge and the capability to conduct neurological assessment. They need to provide holistic and safe care and effectively reduce complications and mortality rate.
