建構加護病房電子交班系統及護理人員接受度之評量

Establishing and Exploring the Intensive Care Unit Computerize Hand-Off System

摘要


醫療不良事件常發生於醫事人員間訊息傳遞缺失,交班對護理人員而言是臨床照護的重要部分,如因缺乏明確的交班工具與流程則會造成訊息的遺漏。本研究目的是為了設計一個適合加護病房護理人員的電子交班系統,並且評價護理人員對此系統的接受度。本研究分為兩個步驟:第一部份是設計結構化的電子交班系統,應用系統開發的螺旋式反覆測試檢驗系統操作時的正確性;第二部分是於系統完成及護理人員接受教育訓練後,以橫斷式問卷調查臨床護理人員對此系統的接受度。問卷採Likert四分法(1-4分),分數愈高表示接受度愈高,共發出140份結果共回收119份問卷,回收率85%。整體接受度平均值為3.05分、護理師對系統的接受度高於契約護士、N2以上的護理人員接受度高於N1與尚未進階之護理人員。建議應發展適合單位結構化交班工具,並加強新進及新調入護理人員交班技巧。

The purpose of this research was to appraise staff's acceptability on a newly designed computerized hand-off system in intensive care units. There were two steps in this research. The first step was to develop a structured computerized hand-off system for critical care units and the second step was to explore staff's acceptability for the hand-off system. The structured computerized hand-off system contains patients individual information, examination results, signs and symptoms, consultation reports, and current status. As for staff's acceptability, a purposive sampling survey was conducted. 119 intensive care unit nurses joined this program. The acceptability rate for the system was 3.05.Nurses with a nursing ladder higher thanN2 had higher acceptance rate than others. Recommendation: Fornewly-hired or newly-transferred nursing staff, we should reinforce their familiarity with this hand-off system in order to facilitate an uniform communication among staffs.
