耳部顯微手術護理指導改善專案

Improvement Project of Nursing Instruction to Ear Surgery Patients

摘要


本專案目的旨在改善護理人員耳部顯微手術護理指導完整率。改善前發現護理人員在進行個別護理指導時,指導內容不一致,使病人對耳部顯微手術護理指導認知偏低。經實地觀察、現況分析發現:缺乏相關護理指導標準及監測工具、護理指導輔助工具不符合現況、護理人員個別且重複指導耗時。故運用文獻查證與決策矩陣分析,擬定解決辦法:建立護理指導標準規範、訂定監測作業、製作多媒體光碟、採用團體護理指導方式等措施。專案實施後,護理人員執行手術護理指導完整率由56%提升至100%,病人對手術護理指導認知正確率由50%提升至96%,病人滿意度由57%提升至99%,有效提升病人手術認知及滿意度,改善護理指導品質。

The aim of this project was to improve the quality of nursing instruction for ear surgery. We have noticed that one-on-one nursing instruction has been inconsistent and this leads to inefficient understanding among the patients. Several problems were found after observations, such as repeated and time-consuming guidance by individual nurse, lack of nursing instruction standards and monitoring tools. Therefore, we used literature review and decision-making matrix methods to create multimedia CD-ROM, group teaching revision, standard instruction establishment, operation monitoring, etc. These resulted in an improvement in the integrity of nursing instruction from 56% to 100%; an increase in patients' general recognition from 50% to 96%, and the patients' satisfaction rate increased from 57% to 99%. This study is not only effective in increasing patients' satisfaction and enhancing recognition toward ear surgery, it also improves the quality of nursing instruction.
