Application of Inter-RAI-HC to Assess the Care Needs of Solitary Veterans Living Alone in Taipei

摘要


本研究採橫斷性調查法,探討台北市獨居榮民健康照護需求。以個人數位助理(PDA)內建MDS-HC評估量表為研究工具,立意取樣大台北12個行政區獨居榮民329人。研究結果發現(1)獨居榮民最常見之慢性病為高血壓,主要健康照護需求包括工具性日常生活活動(Instrumental Activities of Daily Living, IADL)、健康促進、視覺功能、認知、行為、社會功能、用藥管理、精神治療藥物及日常生活活動復健之潛能;(2)個案之年齡、疾病診斷總數、服用藥物總類數可預測其身心問題及社會功能;(3)日常生活活動(Activities of Daily Living, ADL)、工具性日常生活活動、年齡、疾病總數、服藥總數可預測未來入住機構之風險。依據本研究之調查結果,研究者提出建議,針對多重疾病診斷者應有個別性評估追蹤計畫,提供照護以維持功能最佳狀態。

This descriptive and cross-sectional study was designed to evaluate the health needs and issues of elderly veterans living alone in the greater Taipei area. The MDS-HC application installed in a personal digital assistant (PDA) was used as the assessing tool for the collection of 329 cases in our study population.The results showed that hypertension was the most common chronic disease among elderly veterans who lived alone. Hence, their primary health concerns included instrumental activities of daily living (IADL), health promotion, visual acuity, recognition, behavior, social skill, management of medication, usage of psychiatric medication, and ability to return to daily living activities. Age was a critical factor in identifying chronic disease, amount of medication use, and impact on personal social skill. Finally, risk factors of possible hospitalization were considered to be IADL, age, number of underlying diseases, and amount of prescribed medication. To conclude, in order to better assist individuals with multiple diseases, careful planning and close follow-ups are required to yield optimal health care.
