護理指導團體增進失智症病人家屬照顧之成效

摘要

台灣地區於2009年老年人口比例已高達10.6%（內政部，2010，1月21日），大於65歲以上罹患失智症之盛行率為1.7~4.3%，失智症照顧需求倍感殷切；而失智症是一持續惡化疾病，對於照顧者而言，病人併發問題行為及精神症狀常使照顧者受到身心折磨及負荷，易導致照顧者有情緒之問題產生。為提升家屬照顧能力，因而成立專案小組，經特性要因分析後發現有「未有充分時間提供指導」、「家屬對疾病認知不足」、「家屬平日晚間來訪」、「學習場所干擾多」等影響因素並提出對策，修訂護理指導團體時間，規劃護理指導團體作業流程及內容等相關策略之執行，結果：1. 家屬對失智症知識正確率由78.4%提升至98.7%；2. 家屬對護理指導滿意度達到95.8%；3. 醫療團隊成員對護理指導滿意度從35.4%提升至91.2%。本專案之推行獲得家屬與相關失智症團體許多正面回饋，並能有效提升家屬照顧能力及提升護理服務品質。

In Taiwan, the proportion of elderly population in 2009 was 10.6%. The prevalence of those suffering from dementia who were 65 years and older was 1.7% to 4.3%. Dementia is a progressive disease and those with dementia gradually lose the ability to live independently, needing the assistance of others. Caring for an elderly person with dementia is a major challenge in life and it entails emotional, physical, social and financial burdens. Caregivers of patients with dementia can experience various emotional problems during the course of patients' illness. This problem-based project was designed to improve caregivers' care ability. The result of the analysis was to propose a solution to enhance the care ability of caregivers, and thus the establishment of the project. The characteristics of the amendments to the care guidance group time, planning and care guidance group processes and content related to the implementation of the strategy were based on the results of the project: 1) Rate of caregivers' knowledge of dementia, increased from 78.4% to 98.7%; 2) Rate of caregivers' satisfaction with care guidance at 95.8 %; 3) Members of the medical team care guide satisfaction increased from 35.4% to 91.2%. When implementing this project many positive feedback from family members and related dementia groups were received, and thus, this project can effectively enhance caregivers' ability and the quality of care services.
