不同姿位乳房護理對產後婦女乳房腫脹成效評估

The Effects of Breast Care Positioning on Breast Engorgement

The purpose of this study was to compare the effect of two positions-one with semi-fowler's position and the other with supine position in the treatment of breast engorgement. This was a quasi-experimental study conducted in the postpartum ward of a medical center in central Taiwan. The study recruited 60 mothers; 30 in the supine position group and 30 in the semi-fowler's position group. The semi-fowler's position group and the supine position group received breast care about 90 minutes for relieving breast engorgement. The pre-and post breast engorgement scale which includes experience of breast pain, engorgement, hotness and discomfort were recorded. The data were analyzed using descriptive and inferential statistical methods and using the statistical software SPSS 18.0 version. Study results showed no statistical differences between the two groups at baseline. The subjective experience of breast pain, engorgement, hotness and discomfort were statistically different between the two groups at 90 minutes after intervention, whereas the supine position group was found to be more effective than the semi-fowler's position group in relieving engorgement.The findings provided empirical evidence supporting that supine position may be used as an effective position in the management of breast engorgement. By using breast care, nurses can handle breast engorgement problems more effectively in primary care and hence, promote women to success in breast feeding.
