骨科下肢術後病人因疼痛未確實執行復健運動時,容易造成身體活動度受限及影響日常生活的自理能力,甚至發生下肢肌力喪失的問題。本單位2012年因骨科下肢術後病人復健執行率由原來87.6%降為56.8%,故專案目的為提升骨科病人下肢術後復健運動執行率。經現況分析原因為:病人害怕疼痛及不清楚衛教內容、護理人員衛教指導不完整或忘記執行復健運動。解決辦法為製作「骨科術後疼痛衛教單張」、舉辦「下肢復健運動」在職教育與「骨科術後復健運動」團體衛教活動、執行「復健運動稽核單」等;改善後骨科下肢術後病人復健運動執行率由56.8%提升至96.67%。本專案運用衛教單張內容、透過專業教育訓練及落實復健指導之稽核,提升病人復健運動執行率,獲得病人及家屬肯定,並提升護理品質。
Orthopedic patients often fail to perform rehabilitation exercises after lower extremity surgery because of pain and discomfort, causing limited mobility and hampered physical activities and self-care ability, leading to loss of muscle strength. This program aimed to improve the rehabilitation exercise implementation rate in orthopedic patients with lower extremity surgery and maximize the restoration of their lower extremity functions. In 2012, the implementation rate of rehabilitation exercise for patients with lower extremity surgery dropped from 87.6% to 56.8%. Analysis indicated that major cause of the decline included patients' fear of pain, a lack of proper health education, deficient guidance from the nurse, and failure to remember to perform rehabilitation exercises. Solutions for the situation consist of distributing postorthopedic surgery care information pamphlets, expanding in-service educational courses on lower extremity rehabilitation exercise, and utilizing rehabilitation audit forms. During March 1-August 31, 2013, the implementation rate of rehabilitation exercise for orthopedic patients with lower extremity surgery increased from 56.8% to 96.67%. The program employed the use of health education pamphlets, professional education and training services, and rehabilitation audit forms to improve the implementation rate of rehabilitation exercise in patients, and further enhanced the quality of postoperative care.
