顱內動脈瘤破裂病人術後於加護病房期間家庭韌力的經驗:由主要照顧者觀點

Primary Caregivers' Perspective on Resilience of Families of Patients With Intracranial Aneurysm Rupture Undergoing Surgery during Intensive Care Period

摘要


本研究從家庭韌力觀點出發,經由主要照顧者觀點去瞭解顱內動脈瘤破裂病人術後入住加護病房期間,家人的家庭韌力經驗本質結構。以現象學研究方法,於北部某醫學中心神經外科重症病房,共收案10位。資料收集秉持現象學存而不論的態度進行深度訪談,依現象學分析步驟進行分析。以Lincoln及Guba(1985)的四個準則進行嚴謹度的控制。研究結果呈現五個主題:一、秒爭生命存續的醫療;二、確保專業的醫療照顧;三、放心交付護理專業照護;四、共同掌控疾病,規劃家庭的未來;五、凝聚家庭正向力量,共同面對困境。研究發現有助於臨床護理人員面對顱內動脈瘤破裂病人之主要照顧者時,能及早協助啟發家庭韌力,提供身心靈全面照護協助,減緩家庭困境,促進家庭功能早日恢復。

This study investigated family resilience from the perspective of primary caregivers toward a family member with intracranial aneurysm rupture admitted to an intensive care unit. This study used phenomenological methods. Data were collected through in-depth interviews. Participants were 10 caregivers of patients admitted to a neurological intensive care unit of a medical center in Northern Taiwan. No new themes were found in data analysis, and data saturation was achieved. In-depth interviews were conducted by strictly controlling the four criteria proposed by Lincoln and Guba (1985). Five themes emerged from the results of this study: (1) urgent implementation of life-saving medical care, (2) professional accountability of medical care, (3) trust in professional nursing care, (4) shared mastery of illness and planning the future of the family, and (5) cohesion of positive forces among family members and facing difficulties together. These results may aid nurses in immediately assisting patients' families to encourage family unity, provide comprehensive care of the body and mind to alleviate the families' suffering, and recover the family to their previous functional form.
