初產婦的健康識能與職場母乳哺育計畫及能力相關之探討

Relationship of Health Literacy with Breastfeeding Plan and Ability in Employed Women during their First Pregnancy

摘要


本研究目的探討初產婦的健康識能與職場母乳哺育計畫與能力之相關性,以方便取樣選取126位北區某醫學中心初產婦職業婦女為研究對象,納入條件包括:年滿20歲能閱讀懂中文字、生產時仍有專職工作,且有意願產假後返回職場者、足月產、單胞胎。採結構式問卷自填方式,研究工具包括母乳哺育健康識能(包括功能性、互動性及批判性健康識能),職場母乳哺育計畫以及母乳哺育能力(包括母乳哺育問題解決能力、職場母乳哺育態度、職場母乳哺育認知三面向)。結果為產後返回職場的初產婦職場母乳哺育計畫為平均8.8±5個月,相較於選擇育嬰假的初產婦為平均11.0±6個月,顯著較低(p=.04);母乳哺育健康識能中功能性、互動性、批判性健康識能與母乳哺育問題解決能力及母乳哺育態度分別有顯著正相關性,而母乳哺育互動性健康識能與職場母乳哺育認知為顯著負相關;初產婦的母乳哺育健康識能程度越高者,有較正向的母乳哺育態度及母乳哺育問題解決能力。建議未來提供臨床護理人員於臨床實務中能提高初產婦的母乳哺育健康識能,應有助於提倡職場的母乳哺育。

This study, conducted at a medical center in Northern Taiwan, investigated the correlation between health literacy, breastfeeding ability, and job characteristics of employed women during their first pregnancy (primiparas). This study used some data from a cross-sectional study for analysis and then included 126 women. Inclusion criteria comprised age of 20 years old, ability to read and understand Chinese, a full-time job during the pregnancy, and willingness to return to work after maternity leave. Furthermore, participants had to have full-term pregnancies, resulting in a single birth. The data were collected using a structured questionnaire. This study demonstrated that breastfeeding programs in the workplace lasted 8.8±5 months-significantly shorter than the 11.0±6 months of parental leave primiparas generally request (p = .04). The findings also revealed that the degree of health literacy (including functional, communicative/interactive, and critical health literacy) and breastfeeding ability had a good correlation. Breastfeeding problem-solving ability demonstrated a significantly positive correlation with breastfeeding attitude, whereas a significantly negative correlation was found between interactive health literacy and attitude. In conclusion, the higher the level of health literacy, the higher is the motivation for breastfeeding. Therefore, among primipara women, breastfeeding health literacy should be improved and breastfeeding should be promoted in the workplace.
