本研究旨在探討正念減壓課程對癌症病童主要照顧者焦慮、壓力及自我療癒之成效。採類實驗研究設計,研究對象為北部某醫學中心兒童病房入院接受化學治療或放射線治療、18歲以下初診斷或復發癌童之20歲以上之主要照顧者,實驗組及對照組各收集30人;實驗組接受正念減壓課程,對照組則為常規照護。以情境焦慮量表、主觀壓力量表及自我療癒力量表為研究工具,進行正念減壓課程介入前及介入後第4週、第8週之資料收集。研究結果發現,參與研究之全體癌童主要照顧者平均年齡41.56歲(SD=6.53),介入措施前量表平均分數,焦慮程度為50.30(SD=12.76)、壓力程度為27.28(SD=6.71)、自我療癒能力為79.63(SD=15.29)。介入前後各時間點兩組之焦慮程度(p=.461)及自我療癒力(p=.190)雖未達統計上顯著差異;但實驗組的壓力程度在介入後第4週下降且達統計上顯著差異(p=.036),顯示正念減壓課程可降低癌童主要照顧者之壓力程度,故臨床照護癌症病童時,可建議主要照顧者接受正念減壓課程訓練以減輕壓力,緩解心理衝擊及負荷,提升照顧者身心健康狀態及生活品質。
We aimed to evaluate the effect of mindfulness-based stress reduction program on anxiety, stress, and self-healing power in the primary caregivers of children with cancer. The study used a quasi-experimental design and the objects included primary caregivers (more than 20 years old) with children (less than 18 years old) who had newly diagnosed and recurrent cancer and were undergoing chemotherapy or radiation therapy at a children's cancer ward in Northern Taiwan. The intervention and control groups had 30 participants each. The intervention group received mindfulness-based stress reduction therapy, and the control group received routine nursing care. Structured questionnaires, including state-trait anxiety inventory (STAI), subjective stress scale, and self-healing power scale were used to collect data before and after 4 and 8 weeks of the mindfulness-based stress reduction program. Our findings revealed that the mean age of primary caregivers was 41.56 (SD=6.53) years. Before the intervention, for the main caregivers of children with cancer, the average score for STAI was 50.30 (SD=12.76), subjective stress scale was 27.28 (SD=6.71), and self-healing power scale was 79.63 (SD=15.29). Moreover, anxiety (p= .461) and self-healing power (p= .190) between the intervention and the control groups before and 4 and 8 weeks after intervention did not reach statistically significant differences. However, in the fourth week, the degree of stress decreased and reached a statistically significant difference (p= .036) in the intervention group. Thus, the mindfulness-based stress reduction program can reduce stress in the primary caregivers of children with cancer. We recommend that the primary caregivers of children with cancer enroll in the mindfulness-based stress reduction program to improve their physical and mental wellbeing during the clinical care period.
