年輕型腦中風病人初次罹病經驗之質性研究

First Stroke Experience of Young Adults: A Qualitative Study

摘要


本研究旨在探討年輕型腦中風病人之初次罹病經驗。採質性研究以立意取樣,於2020年3月1日至11月4日在某醫學中心神經內科病房共收錄28位年齡介於20-45歲初次罹病年輕型腦中風的病人,以現象學研究法深入訪談,採Colaizzi's(1978)七大步驟歸納分析出四大主題:一、輕忽病兆;二、擔心病情;三、健康型態自我覺醒;四、親情關懷與陪伴。年輕型初次中風病人以栓塞為多,屬輕度中風,治療後即可出院,由於年輕人較易淡忘病痛,失去自主健康管理警覺,故預防二次中風,指導病人遠離中風危險因子,判斷中風病兆,居家自我健康保健等為意旨。本研究期望提醒年輕型腦中風病人重視中風預防,勝於治療,並有助於臨床護理人員及時提供年輕型腦中風病人生心靈的全人照護,促進病人身心康復、回歸正常生活。

This study investigated the initial experiences of young patients with stroke. Between March 1 and November 4, 2020, 28 patients aged 20 to 45 years who had experienced stroke for the first time were enrolled in the neurology ward of a medical center. The study was conducted qualitatively and employed suggestive sampling. In-depth interviews were conducted using a phenomenological research method. The Colaizzi's (1978) method was adopted to analyze and summarize four themes: (1) ignoring signs of illness, (2) worrying about illness, (3) healthy patterns of self-awakening, and (4) family care and companionship. Young patients experiencing their first stroke primarily experience embolism, which is classified as a mild stroke. After treatment, they can be discharged from the hospital. However, young patients tend to forget the pain and fail to maintain health management strategies. Therefore, prevention of a second stroke, guidance of patients away from stroke risk factors, diagnosis of stroke signs, and home health care are essential. Clinical nursing staff should provide young patients with stroke with timely whole-body and mind care to facilitate their physical and mental recovery and their return to normal life.
