2019年本單位1-3歲兒童佔住院總人數的49.4%,其中97.5%幼兒在靜脈注射時出現重度疼痛情況,故成立專案小組,分析原因為護理人員注射技巧不純熟、幼兒過去有不好的注射經驗、家屬不知如何安撫、無轉移注意力物品、兒科靜脈注射流程不完整。專案目的為1-3歲住院幼兒靜脈注射重度疼痛率由97.5%下降至36%,專案執行期間自2019年5月1日至2019年12月31日,改善策略包括提升人員靜脈注射訓練、落實注射前給予治療性遊戲、提高家屬正向靜脈注射認知、建置歡樂靜脈技巧之注射治療室、完成修訂兒科靜脈注射流程等。經由執行對策後,靜脈注射重度疼痛率由97.5%降至13.0%,建議靜脈注射前給予治療性遊戲、局部塗抹2% Lidocaine Jelly及改善醫療環境設備,給予病童及家屬的正確的靜脈注射概念,提升照顧品質。
In 2019, children aged 1-3 years accounted for 49.4% of the total number of hospitalizations in our unit. Among them, 97.5% of the children experienced severe pain during intravenous injections. Therefore, a task force was established for examining the causes of this severe pain. The analysis revealed the following reasons: poor injection skills of nurses, children's past bad injection experiences, family members not knowing how to comfort the child, no distracting objects being used, and incomplete pediatric intravenous injection procedures. The purpose of the present project was to reduce the rate of severe pain associated with intravenous injection in 1-3-yearold hospitalized children from 97.5% to 36%. The project implementation period was from May 1 to December 31, 2019. The improvement strategies included the following: training personnel in administering intravenous injections, implementing therapeutic games before injections, improving family cognition to ensure positive intravenous injection experiences, building a comfortable intravenous injection treatment room with a positive atmosphere, and completing revision of the pediatric intravenous injection process. After implementing the countermeasures, the rate of severe pain associated with intravenous injection decreased from 97.5% to 13.0%. It is recommended to implement therapeutic games, topically apply 2% lidocaine jelly, and improve the medical environment before intravenous injection so that the correct intravenous injection procedure is used with pediatric patients, improving quality of care.
