本文是一位骨髓增生不良症候群病人接受異體移植的護理經驗,護理期間自2019年1月6日至1月29日,運用整體性評估病人健康問題為潛在危險性感染、口腔黏膜障礙及焦慮。透過QR Code帶入衛教單張、強化遵守標準防護原則,預防感染發生;由營養師介入調整飲食型態及教導家屬烹調食物方法,改善口腔黏膜潰瘍;主動關懷傾聽,與心理師團隊合作運用抒壓活動,轉移病人焦慮問題;返家居家衛教提供「愛料理」、「藥丸提醒」App加強飲食攝取與遵從服藥的重要性。建議日後可將「移植」字眼建置於健保卡及醫院電腦內,以利病人就醫時能快速辨識,以維護病人安全,冀望此護理經驗作為護理人員參考。
This paper describes nurses' experience caring for a patient with myelodysplastic syndrome who received allogeneic transplantation. Through holistic assessments, we identified the main health problems, namely potentially dangerous infection, oral mucosa disorder, and anxiety, from January 6 to 29, 2019. We provided health education tools through quick response codes to ensure compliance with standard protection principles and to prevent infection. Nutritionists intervened to adjust the patient's dietary patterns and teach the patient's family members to cook foods that would improve oral mucositis. The nurses used active caring and listening, worked with a team of psychologists to develop stress-relieving activities for the patient, and helped the patient use applications for cooking and medication reminders to convey the importance of nutrition and taking medicine properly before discharge. Health insurance cards and hospital computers should indicate patients with transplants for quick identification during doctor visits and to ensure their safety. The results of this study can be used as a reference for nursing staff.
