本文描述一位初次診斷第一型糖尿病學齡期女童,因酮酸中毒入院之照護經驗。筆者於2020年4月29日至5月8日照護期間,經由觀察、會談及羅氏適應模式進行整體性評估,確立其有健康維護能力改變/與初診斷第一型糖尿病有關、恐懼/與害怕測量血糖和施打胰島素有關、危害性家庭因應能力/與病童罹患第一型糖尿病對其疾病知識不足有關等健康問題;筆者首以符合兒童發展年齡所設計的繪本及使用淺顯易懂文字做為溝通策略,再輔以治療性遊戲及正向鼓勵,有效協助病童克服恐懼,達到病童於出院前能自己正確執行血糖監測及胰島素注射治療之目標;另運用跨領域團隊合作模式,提供介入與資源,達到協助案母具備病童居家照護能力之目標,期盼經由此照護經驗分享,作為日後照護之參考。
This article describes the nursing experiences of a school-age girl with a recent diagnosis of type 1 diabetes who was hospitalized due to ketoacidosis. The period of nursing care was from April 29 to May 8, 2020. The patient's medical information, physical assessment, and interview records were collected and evaluated according to Roy's adaptation model, and ineffective health maintenance, ineffective family coping, and fear were identified to be primary health concerns. During the period of hospitalization, the author employed a communication strategy featuring the use of age-appropriate picture books, therapeutic games, and positive reinforcement to manage the patient's fear and cultivate their ability to engage in diabetes self-management. Additionally, teams from various disciplines worked together to help the patient's mother better take care of the patient at home. It is hoped that our nursing experience can be used for care interventions in the future.
