壞死後肝硬化病患的營養評估

Optimal Nutritional Indexes in Post-necrotic Liver Cirrhosis

One hundred and thirteen post-necrotic liver cirrhosis patients (84 males and 29 females) were studied to evaluate their nutrition status. The results revealed that anthropometric parameters, albumin, clinical signs and symptoms, dietary intake decreased while degree of disease severity increased. Linear regression analysis also demonstrated that MAMC (midarm muscle circumference) was correlated significantly with serum albumin level only in male patients without water retention (p<0.01). Then, we compared the nutrition status and the degree of liver disease severity in 10 male post-necrotic liver cirrhosis patients at first hospital admission and 3 months later or more. Although serum concentration of albumin, bilirubin and prothrombin over time did not significantly change alone with liver disease severity progression, clinical signs and symptoms increased, dietary intake decreased. BMI, Percent of usual body weight and percent of body weight change were influenced by water retention. MAC (mid arm circumference) decreased significantly (p<0.05) revealed obvious body mass depletion. Therefore, albumin is an adequate nutrition assessment parameter in male without water retention, mid arm anthropometric parameters seem to be a sensitive nutritional index in post-necrotic liver cirrhosis patients.
