北基地區醫院急診能量評鑑

Categorization of Hospital Emergency Services in Taipei Area

摘要


隨著社會的進步,意外傷害日趨增多,急救醫療服務,勢將成為衛生行政中最重要的業務之一。北基地區為我政治、經濟重心所在,工商繁榮,人口集中,醫療機構質量在臺灣地區可謂首屈一指,惟至今仍乏有效的急救醫療體系;欲謀建立此類系統,最迫切需要完成的工作,即為「急救醫療設施之評鑑」。本研究即係針對北基地區所有172所醫院的急診能量試行評量,以了解急診資源概況,供作規劃依據。根據評鑑結果,臺北市有十二所醫院,得列為一級急救醫院,另有十三所列為二級急救醫院,可用以佈署臺北市之初期急救醫療系統。本研究同時設計出一套合乎我國醫院概況之急診能量評鑑標準,略事增刪後,應可供有關單位,作為日後正式評鑑之用。

As do most areas, Taipei has lacked a functional and efficient system of emergency services. In 1977, financial supported by National Science Council, a survey and categorization of the hospital emergency capabilities in Taipei area (Taipei City, Taipei County and Keelung City) was conducted.The study focused on the single area of hospital emergency care facilities covered (1) emergency department organization and equipment (2) medical staffing of the department and of the hospital in general (3) support services directly related to the emergency department, such as laboratories and X-ray facilities (4) special care units, such as intensive care units (5) experience as reelected by a hospital’s bed size and daily visits and (6) administration policy as it relates to the emergency department.Initially 172 hospitals were studied, of these, 154 (or 89.5%) were found to have emergency departments. These 154 were surveyed in depth and thus form the bases of this report.General characteristics of the hospitals showed the 102 (58.7%) of the 172 have a bed capacity of less than 40 while only 16 (9.3%) have more than 200 beds.During 1977 there were 392,000 emergency department visits to 154 hospitals, nearly 65.7% of the hospitals averaged fewer than 5 daily visits, while 10 hospitals had more than 20 daily visits. Taiwan University Hospital and Veterans General Hospital received 15.9% of the area's entire emergency visits.Of the emergency departments surveyed, 132 of hospitals had neither a department director or committee nor a policy and procedure manual. Only 853 hospitals had a separate emergency entrance, accessibility from the street. 137 of the departments did not have a defibrillator, and 72 did not have a electrocardiogram.An estimate of number f emergency cases that each hospital could handle at one time was made. 33 were believed to handle only two, and 58 were able to handle 20 or more simultaneously.Categorization of emergency capabilities based on the information the study gathered concerning staffing and equipment in hospitals was performed according to set of suggested guidelines. Only 18 (11.8%) hospitals have been categories as comprehensive emergency service.The finding mentioned above and others elicited in the study have proved Taipei area with a fairly comprehensive picture of its existing emergency facilities. Hoping that city officials could utilize these data to develop a regional plan for the metropolitan area to utilize current facilities more efficiently.
