Kanamycin-induced Nephrotoxicity in Nephrotic Syndrome with Spontaneous Peritonitis

康黴素在腎病症候羣合併自發性腹膜炎患者引起之腎臟毒性-病例報告

摘要


Aminoglycoside類抗生素對腎臟之毒性,已被確認,然而大多在實驗的動物身上,發現證據,鮮有人類腎臟毒性證據的報告。早期的研究,以為只有在大劑量之下,才引起腎臟毒性。在今日已經有許多的實驗證實,即使是小劑量的aminoglycoside類抗生素,便可導致腎臟顯微組織形態的變化。吾人報告的這個病例,在腎功能正常,而全身水腫的狀況下,每天僅注射一公克的康黴素,連續注射七天。停止注射一星期後,施行腎臟活體切片,在電子顯微鏡下,發現典型的myeloid body存在於近側曲小管細胞之secondary lysosome內,此即為aminoglycoside類抗生素對腎臟毒性的最直接證據。

In human beings, aminoglycosid-induced nephrotoxicity has been found to be infrequent, especially in case of nephrotic syndrome with spontaneous peritonitis. The case in this report received kanamycin 1.0 gm daily only f or 7 days under the normal renal function conditions but in the edematous state. Renal biopsy was done one week after discontinuance of kanamycin. Ultrastructural examination disclosed secondary Iysosome with myeloid bodies within the proximal tubular cells: an evidence of aminoglycoside-induced nephrotoxicity.
