阻塞性肺部疾病患者血中茶鹼有效治療濃度之研究

Effective Therapeutic Theophylline Level Study in Airway Disease

摘要


本研究是為國人阻塞性肺部疾病患者使用茶鹼之劑量提供依據。我們以高效率液相層析紫外光測定法及酵素免疫分析法檢測血中茶鹼濃度,二法呈明顯之正相關,相關係數r=0.987。而後以高效率液相層析紫外光測定法,測定血中茶鹼濃度,來分析各種阻塞性肺部疾病患者使用氨茶鹼後之藥物動力學變化,進而求出負載劑量及維持劑量。若以達到血中茶鹼濃度10μg/ml估計,在不吸煙之無症狀哮喘患者,其負載量約為6.42mg/kg,維持劑量是0.73mg/kg/hr。慢性阻塞性肺病患者負載劑量為5.38mg/kg,維持劑量是0.75mg/kg/hr。慢性阻塞性肺病併發肺心症患者負載劑量約為4.20mg/kg,維持劑量是0.56mg/kg/hr。故我們建議茶鹼劑量的使用宜按本實驗結果作一基線,並配以經常的監測血中濃度才恰當。

High pressure liquid chromatography with UV detection (HPLC-UV) was used to compare with enzyme multiple immunoassay technique (EMIT) in serum theophylline concentration determination. Both methods showed positive correlation with correlation coefficient 0.987 in 80 cases of our techniqual control group. We used HPLC-UV method then to detect theophylline pharmacokinetic parameters in 26 cases of airway disease patients. Our data indicated in nonsmoker asymptomatic asthma patients, the loading dose is 6.42mg/kg, maintenance does 0.78mg/kg/hr; in COPD patients, the loading dose is 5.38mg/kg, maintenance dose 0.57/mg/kg/hr and in COPD with cor pulmonale patients, the loading dose is 4.20mg/kg, maintenance dose 0.56mg/kg/hr. The results suggested the theophylline dosage should be adjusted on the basis of frequently obtained plasma concentration.
