組織擴張之臨床運用

軟組織缺損的重建，傳統的治療包括植皮，皮瓣、肌肉瓣或肌皮瓣的轉移（rotation），前移（advancement），換位（transposition）或顯微游離式移植（microvascular free transfer）。不論運用任何方法都會有其限制或缺失。在整形重建外科方面，組織擴張（soft-tissue expansion）是一種新且簡單的手術方法。與傳統的重建手術截然不同的是：它不會引致供給區（donor site）的任何缺損或明顯的障礙。我們成功地利用該項技術，治療一位背部有一大區域疼痛性凹陷瘢痕的年輕病例。由於此一新技術在國內尚無相關的文獻報告，而且在日後的整形重建外科方面，它具有無可限量的發展與運用價值。因此特別提出報告，以供參考。

In the reconstruction of soft-tissue defect, traditional grafts and flaps such as rotation, advancement, or transposition of cutaneous, muscular, or myocutaneous flaps, or microvascular free transfer of flap sometimes provide an unaesthetic color and texture match, therefore donor-site scars and flap reliability may be significant problems. Soft-tissue expansion is a versatile new technique which offers the potential for reducing the technical complexity of some flap procedures and increasing flap reliability. The color, texture, and hair-bearing quality are all preserved from the donor tissue. By using this new technique we have successfully treated a young male with a large area of painful, depressed scar over his back. There is no correlative report being presented in literature domestically. The ultimate scope and limits of this technique are as yet undefined.
