The ß-Lactam Antibiotic Resistance of E. Coli Isolated from Clinic

臨床之大腸桿菌對β-lactam類抗生素抗藥性之研究

摘要


由三軍總醫院分離的102棟大腸桿菌中發現有76.5%(78/102)是屬於Ampicillin抗藥性表現型菌株。若以β-Iactamase試紙偵測法檢測上述抗藥性表現型菌株時,有97.4%(76/78) 是呈陽性之酶活性反應,且這其中有76.3%(58/76)可經由菌落DNA雜交法測知含有TEM-1或TEM-2 β-Iactamase之基因。有兩株Ampicillin抗藥性表現型菌株不產生β-Iactamase,其可能由其他抗藥機轉逃避Ampicillin之毒殺。在102株菌中僅發現一株具有盤尼西林醯酶活性,但此菌株卻對Ampicillin敏感,此事實顯示某些細菌雖可以盤尼西林酼酶切除β-Iactam類就生素之側鏈結構但卻未能對該類抗生素產生抗藥性。由尿液檢體所得之33株大腸桿菌中,有70%(23/33)是屬Ampicillin抗藥性表現型,其中96%(22/23)可產生β-Iactamase但僅68.2%(15/22)菌株帶有TEM-1或TEM-2β-lactamase基因。此事實指出除了TEM-1和TEM-2之β-lactamase基因可負責合成β-Iactamase外,可能尚有其他基因也能合成β-Iactamase。

Of 102 Escherichia coli isolates obtained from the Tri-Service General Hospital in Taipei, 76.5% (78/102) were found to be resistant to ampicillin. Among the said ampicillin-resistant isolates, 97.4% (76/78) were found to show ß-Iactamase activity, as detected by the method of ß-lactamase detection papers, and 76.3% (58/76) of ß-Iactamase producing E. coli isolates were found to possess the TEM-1 or TEM-2 β-lactamase gene when examined by colony hybridization. However, there were two (about 2%) ampicillin-resistant isolates possessing no ß-Iactamase activity. These two isolates might somehow escape the killing action of ß-Iactam antibiotics by other means. There was on1y one (about 1%) isolate which reflected the penicillin acylase activity but was sensitive to ampicillin, indicating that penicillin acylase might play a negligible role in the resistance to ß-Iactam antibiotics. Of 33 E. coli isolates which were sourced from urine specimen, about 70% (23/33) were found to be ampicillin resistant, and 96% (22/23) of them showed ß-Iactamase activity. However, on1y 68.2% (15/22) of the ß-lactamase producing strains contained TEM-l or TEM-2 ß-lactamase gene.
