A 22 years old female suffered from a car accident with multiple injuries and head contusion. She received emergency transfusion of 6 units of whole blood in the United States. Antibody screening revealed autoantibody and four alloantibodies, namely, anti-E, -Jk^a, -K and -Kp^a. Alloantibodies were induced by emergency transfusion. K and Kp^a antigens are virtually unknown in Chinese populations and anti-K and -Kp^a must be induced by a blood transfusion from Caucassians or Blacks, Multiple alloantibodies identification is a challenge to blood bank and autoantibodies add difficulty in trouble shooting. The immunohematological condition of this special case is analysed and discussed.
