雙胞胎新生兒臨床分析

根據文獻上各國的統計，雙胞胎的發生率有極大的差異（0.7－4.5%），以黑人較高，東方人較少，而白人居中。雙胞胎新生兒的平均妊娠週數、體重，皆較單胎妊娠為低，而其週產死亡率及罹病率均高於單胎。回顧空軍總醫院自民國71年1月至80年12月間的雙胞胎出生資料，在15550生產次數中有109對雙胞胎（0.7%；1/143）。此109對雙胞胎的性別分佈：同為男性占52對，同為女性占42對，其餘15對為不同性別；男女嬰總數比為1.2/1。平均妊娠週數為37(6/7) ± 1(1/7)週（mean ± SD），其中45%少於38週。平均出生體重2592 ± 48 g，其中44.5%小於2501 g。平均身長46.8 ± 2.8 cm，平均頭圍32.0 ± 1.6 cm，平均胸圍30.5 ± 1.8 cm。生產方式主要以剖腹產占多數（62.4%）；次為自然生產（17.4%）；真空吸引術及產鉗分別為15.6%及4.6%。此109位孕婦的併發症有早期破水6例，高血壓1例，發燒1例，子癇前症4例，沒有母親死亡。嬰兒的併發症除早產及低出生體重外，有12例疑似胎兒對胎兒輸血症候群，有1對雙胞胎因極度早產死亡。

Twining is one of the most interesting events occurring in human medicine. Twin pregnancies are with significantly higher morbidity and mortality than singleton pregnacy. From January 1982 to December 1991, the general data of physical examination were collected retrospectively for all available newborns from the nursery of Air Force General Hospital. There were totally 15441 singleton pregnancies and 109 twin pregnancies. The incidence of twins is 0.7%. There were 52 pairs of male twins, 42 pairs of female twins, and the other 15 pairs of unlike sex twins; the male to female ratio is 1.2/1. The mean gestational age was 37(6/7) ± 1(1/7) weeks (mean ± SD); 45% less than 38 weeks. The mean birth body weight was 2592 ± 48 g; 44.5% less than 2501 g. the mean head circumstance was 32.0 ± 1.6 cm; the mean body length was 46.8 ± 2.8 cm. The delivery types of twins included CIS (62.4%), NSD (17.4%), EVE (15.6%), and ELF (4.6%). The maternal complications included 6 PROM; 1 hypertension and 1 maternal fever. There was no maternal mortality. Forty-five percent of twins were premature delivery, 44.5% of twins were with birth body weight less than 2501 g. Twin to twin transfusion syndrome was suspected in 12 cases. Only one pair of twins expired because of extreme prematurity.
