Transesophageal Echocardiogram in the Dignosis of Traumatic Transection of the Thoracic Aorta
主動脈創傷性斷裂－利用經食道心臟超音波診斷之經驗
摘要
鈍挫傷造成主動脈斷裂是屬於致命的傷害。儘早診斷及適當的治療，是使病人存活下的關鍵。本文報告自1990至1992年之三年間，三軍總醫院利用經食道心臟超音波，診斷創傷性主動脈斷裂的三個案例，第一例是慢性病例，診斷主要依賴胸部電腦斷層，主動脈造影術，及經食道心臟超音波，其餘兩例為急性案例，主要診斷依據胸部電腦斷層或經食道心臟超音波，特別是最後一例，當時臨床症狀懷疑主動脈斷裂，但胸部電腦斷層無法確定。在手術室中利用經食道心臟超音波迅速診斷出主動脈斷裂。我們的結論是經食道心臟超音波在胸部主動脈創傷性斷裂的診斷是一種非常有價值的工具。

Traumatic aortic rupture is a lethal injury. Early diagnosis and prompt treatment is the cornerstone of survival. We report our experience in the diagnosis of traumatic aortic transaction with transesophageal echocardiography (TEE) from 1990 to 1992. Three cases of transaction of aorta were diagnosed and treated at Tri-Service General Hospital in this period. The first one was a chronic case, diagnosis was made by computed tomography, aortogram, and TEE. The diagnosis of the other two cases was based on computed tomography and/ or TEE only. Particularly, the last one was quickly diagnosed by TEE at operation room, while clinical features were obscure, and computed tomography was not able to make a diagnosis. In conclusion, TEE is very valuable in making a diagnosis for traumatic transaction of the thoracic aorta.
