Descending Necrotizing Mediastinitis-Case Report
下行壞死性縱膈腔炎－病例報告
摘要
下行壞死性縱隔腔炎是一種由口咽部感染向下蔓延至縱隔腔所引發的縱隔腔炎。診斷經常被延後確定，單純頸部切開引流及抗生素治療常無法控制病情，必須經開胸手術或經劍突下進行縱隔腔膿瘍之清除及引流；死亡率偏高；使用電腦斷層攝影檢查能提早診斷及提供膿瘍所在之位置。本文報告一例由扁桃腺周圍膿瘍所引發之下行壞死性縱隔腔炎，經頸部切開及開胸手術引流縱隔腔膿瘍後成功治療之病例。

Descending necrotizing mediastinitis (DNM) is an uncommon mediastinitis characterized by infection arising from the oropharynx spreads to the mediastinum. The diagnosis of DNM is almost always delayed. Drainage of the superior mediastinum abscess through cervical incision is usually inadequate. Aggressive mediastinal drainage and diagnostic computerized tomography are very important. We report one patient with infection arising from peritonsillar fossa descends to the superior mediastinum. Completely recovery was achieved after cervical incision and left side thoracotomy debridement and drainage.
