摘要

澎湖地區人口，以年輕的駐軍戰士及年長的農、漁民為主，這兩族群即是疝氣好發之年齡層。本院自1992年7月至1994年6月進行“成人鼠蹊部疝氣（Marlex人工網膜無張力修補手術之研究）”。全部74人（77側）疝氣手術中，34人（36側）接受人工網膜Marlex修補：40人（41側）用Bassini手術修補。結果：在Marlex組中合併症3例（8.8％）；1例併發傷口血漿腫，1例術後神經痛，及1例疑似排斥反應。在Bassini手術組合併症9例（22.5％）；2例併發傷口感染，2例術後神經痛，1例輸精管損傷，及2例併發術後陰囊水腫。瘀血，有1例治療後合併睪丸炎。經過24個月期間的追蹤，發現Marlex組在術後併發症，疼痛及恢復時間有較滿意之結果；在大於45歲組具有較高的合併症（30.5％）。本計劃為無選擇性採用Marlex做為疝氣修補。此一簡單無張力修補手術可減少張力，疼痛，及手術合併症；加上它的有效性及安全性，不失為一種疝氣修補手術之選擇。

In Peng-Hu, the general population is primarily younger soliders, farmers and fishermen. These bimodal age distribution is the high risk groups of groin hernia. From July 1992 to June 1994, total 77 inguinal hernia repair in 74 adult patients were collected and analysed. Thirty-four were repaired with Marlex mesh, unselectively, the remaining forty cases were repaired with Bassini's procedure. Postoperative complication, at Marlex group, was found in 3 cases (8.8%). The higher complication rate was noted in the group of age more than 45 years. There is no statistical difference between the two groups in terms of operation time and recurrence. The hernia repair with Marlex can reduce suture line tension, and postoperative wound pain. So the procedure may be used alteratively in the adult cases after more extensive study have been presented.
