三度空間注射顯影劑之磁振血管攝影術於畸形吞嚥困難的診斷

摘要


三度空間注射顯影劑之磁振血管攝影術是最近幾年迅速發展的非侵襲性血管造影技術,為藉由靜脈注射顯影劑使血液T1時間縮短並結合梯度迴訊磁振造影技術,產生類似傳統導管血管攝影術呈現的血管影像。迷行右鎖骨下動脈壓迫食道造成的畸形吞嚥困難在這攝影術上的表現與臨床運用,目前文獻上尚未有人報告。我們提出這樣一個畸形血管造成吞嚥困難的病例,一開始表現像是食道有腫瘤壓迫,經三度空間注射顯影劑之磁振血管攝影術不僅能清楚地顯示迷行右鎖骨下動脈的整個血管與周圍主動脈弓分支間的關係。我們希望藉此病例喚起大家的注意,留意畸形血管造成吞嚥困難的可能性及三度空間注射顯影劑之磁振血管攝影術在這類病患的運用,並能適時地給予適當的治療。

Three-dimensional contrast-enhanced magnetic resonance angiography (3D CE-MRA) was performed for evaluation of an aberrant right subclavian artery (ARSCA) associated with dysphagia lusoria in a 73-year-old man. The rare syndrome and the usefulness of 3D CE-MRA for this application are discussed.
