應用中文化中風量表評估急性缺血性腦中風患者可信度之研究

摘要

將美國國家衛生研究院中風量表及國內常用之Glasgow昏迷指數量表、巴氏量表和雷氏修正量表經四位專家中文化後，由四位神經科醫師分別於急性缺血性腦中風患者入院時及入院24小時後進行二次評估，共計評估35位患者，以瞭解組間和組內的可信度，四種中文量表問之一致性，和評估不同中風類型問可信度的差異性。結果顯示中文化的美國國家衛生研究院中風量表、Glasgow昏迷指數量表、巴氏量表、雷氏修正量表的肯德爾和諧係數分別介於0 .43 ~ 0.92、0.70 ~ 0.85、0.53 ~ 0.91、0.85 ~ 0.90間。上述四種量表組內可信度的斯皮爾曼等級相關係數平均值皆大於0.75。美國國家衛生研究院中文化中風量表與其它三種量表間的絕對相關值皆大於0.8以上。不同類型中風間可信度的差異性則不大。因此這四個中風量表中文化後很適用於未來評估及追蹤急性缺血性腦中風國人的臨床病程。

All of the national institute of health stroke scale, Glasgow coma scale, Barthel index and modified Rankin scale were translated into Chinese stroke scales by four specialists. A total of 35 patients with acute ischemic stroke were evaluated by four neurologists, using the above four Chinese scales, to test the interrater and intrarater reliabilities, the correlation among these scales, and the reliability among the stroke subtypes at entry and 24 hours after admission. It presented that the Kendall coefficient of concordance among the Chinese versions of national institute of health stroke scale, Glasgow coma scale, Barthel index and modified Rankin scale were 0.43 - 0.92、0.70 - 0.85、 0.53 - 0.91、0.85 - 0.90. Each mean value of intrarater Spearman rank correlation coefficient within four Chinese scales was larger than 0.75. Each absolute value of Spearman rank correlation coefficient between the Chinese national institute of health stroke scale and the other Chinese stroke scales was more than 0.8. There were no marked difference in interrater and intrarater reliabilities using the four Chinese stroke scales among the stroke subtypes. Moreover, the four Chinese stroke scales were suitable to evaluate and follow up the clinical course of acute ischemic stroke in Chinese.
