Treatment of Permanent Atrial Fibrillation with Concomitant Cardiac Surgery

Background: The Cox Maze III procedure is an effective surgical method for eliminating atrial fibrillation (AF) and restoring atrial contractility. New surgical methods for ablation of AF use alternative energy sources, such as radiofrequency and microwave radiation. We assessed whether these methods could attain the same success rate as the Cox Maze procedure. Methods: Between August 2002 and July 2003, 13 patients (7 men and 6 women) presenting with different forms of pathologies and chronic AF were studied retrospectively. Their mean age was 64±8.3 years (range 42-75). The left atrial maze procedure followed the concept of Cox Maze III, using either radiofrequency or microwave energy, and patients underwent valve replacement/repair and coronary bypass grafting. Results: The Cox Maze III procedure took longer than either radiofrequency or microwave radiation (P<0.05). The Maze procedure, and radiofrequency and microwave radiation restored sinus rhythms in 100%, 75%, 86% of the patients, respectively. There were three complications: atelectasis, sick sinus syndrome, and post-cardiotomy syndrome. The patient with sick sinus syndrome needed a pacemaker. Conclusions: The combination of complex valve surgery and the Maze procedure was safe. Microwave or radiofrequency ablation is a safe and highly efficacious treatment for permanent AF.
