Z-plasty and Augmentation Mammaplasty for Correction of Severe Breast Deformity Caused by Abscess: Report of A Case

摘要

乳房的變形對病人造成美觀及心理上的許多問題。治療方針在於重建一個美觀且對稱的乳房，尤其是另一邊的乳房未受到影響時特別重要。乳房膿瘍並不少見，但很少產生乳房嚴重的變形。在此，我們報告一位33歲女性病患，因左側乳房膿瘍，於兩年前接受了兩次引流手術，卻造成了左側乳房容積縮小，外觀變形及乳頭乳暈位移的情形。我們將填充了200cc生理食鹽水的人工彌補物，放入左胸大肌下，來達成對稱的乳房容積，再以一個z-Plasty來矯正乳頭乳暈的位移，手術結果十分令人滿意。據我們所知，這是第一個因乳房膿瘍產生之嚴重乳房變形而需手術整型的報告病例。

Breast deformity imposes great cosmetic and psychological problems on the inflicted patients. The treatment goal is to achieve an aesthetic and symmetric breast contour, especially when the other side is unaffected. Breast abscess is not uncommon, but rarely causes severe deformity. We present a 33-year-old woman treated with zplasty and augmentation mammaplasty. The patient had undergone received surgical drainage twice for her left breast abscess two years before, which led to breast volume deficiency, contour deformity and malposition of the nipple-areola complex. A 200 cc saline-filled prosthesis was implanted in a subpectoral pocket to build up a symmetric breast mound and z-plasty was performed to correct the trapdoor deformity and the malposition of the nipple-areola complex. The surgical result was satisfactory. To the best of our knowledge, this is the first report of severe breast deformity secondary to previous breast abscess that required reconstructive surgery.
