十二指腸腺鱗狀癌: 病例報告
摘要
消化道惡性腫瘤中，源自於小腸的約占百分之一。小腸的惡性腫瘤中，以腺癌為最常見，其次為類癌、淋巴癌及惡性肉瘤。這里提出一例這種罕見的小腸癌。但回顧英文文獻，只有五篇混合型皮癌一腺鱗狀癌的個案報告。我們推測其組織發生機轉，可能是小腸中未分化的基底細胞於惡性變化時，產生了雙向的異生而造成，其治療目前苭H手術為主。至於預後，在國外的病例報告中皆不理想，術後平均存活時間只有數個月。

Small intestine malignancies are rare among gastrointestinal cancers, the most common histologic type of small intestine malignancies being adenocarcinoma, followed by carcinoid, lymphoma and sarcoma. Here we report an exceedingly rare case adenosquamous carcinoma (ASC) of the small intestine with only five case reports in the English literature. The main interest of the tumor is its pathogenesis from the glandular mucosa of the small intestine and its clinicopathologic behavior to the clinicians. Because of its very rare incidence, the true prognosis remains unclear. As aggressive clinical course was noted in the case reports, however, surgical resection seems to remain the treatment of choice.
