原發性大腿化膿性肌炎：二例病例報告

摘要

化膿性肌炎在非熱帶地區並不常見，對於這些地方的臨床醫師而言，診斷與治療並不容易。本文報告兩例原發性大腿化膿性肌炎的案例。一位是26歲的肥胖女性，一位是63歲的糖尿病男性患者，兩人因單側大腿腫痛3-4週而求診。兩人皆無近期手術或外傷的病史，僅呈現發燒、白血球及發炎指數皆昇高的臨床現象。經過超音波、核磁共振及核醫檢查而確診爲大腿化膿性肌炎。治療上，採取切開引流及多次擴創術，抗生素使用共達6-7週。兩人皆達到完全性的治療，無任何併發症。

Primary pyomyositis is an uncommon disorder in non-tropical climates and represents a unique diagnostic and therapeutic challenge to the clinician. A 26-year-old obese female and a 63-year-old male with poorly controlled diabetes mellitus, both suffered from painful unilateral thigh swelling for the previous 3 to 4 weeks. Fever, leukocytosis, and high inflammation indices were noted. The two patients had no recent history of trauma or surgery. Ultrasonography, magnetic resonance imaging, and a gallium-67 scan of the inflamed area revealed abscess formation on the anterior and lateral aspects of the thigh. Immediate incision and sequential debridements were performed. Overall, they were administered antibiotics for 6 to 7 weeks and recovered without any complications.
