Minimally Invasive Esophagectomy in Treatment of Esophageal Cancer

微創食道切除手術治療食道癌

摘要


微創食道切除手術是一種要求高度技巧的手術,雖然在文獻中被認爲是一種合理而安全的食道癌治療方式,但是與傳統開胸與開腹食道切除手術相比,對減輕週術期的併發症與長期的存活是否有額外的好處?目前則未可知。然而,在一些文獻結果包括我們在台大醫院的經驗,顯示這種治療方式,一旦術者本人超越過學習的階段,仍然會有令人滿意的結果。儘管如此,此種方式在未來仍然需要一些研究來比較以各種不同方法施行微創食道切除彼此問之優劣,以及這些手術對病患長期存活的確切影響。

Minimally invasive esophagectomy (MIE) is a technique-demanding surgery. Although the procedure has been demonstrated to be feasible and safe for treating esophageal cancer, its additional benefit in comparison with traditional open surgery has not been confirmed in terms of its perioperative and oncological results. Nonetheless, the results of several authors including ours at the National Taiwan University Hospital have demonstrated that the MIE approach can result in a rather satisfactory perioperative results once the learning curve has been overcome. Further studies are required to compare the results of various approaches of MIE and to examine their impact on the long term survival of patients with esophageal cancer.
