Necrotizing Fasciitis of Chest Wall Caused by Pathogenic Eikenella Corrodens in Pediatric Empyema Thoracis: Report of a Case

Eikenella Corrodens(侵蝕艾肯菌)是一種正常的口咽部菌種,但有可能在人類咬傷處或頭頸部傷口造成感染。但不論在成人或兒童,它極少在肺部感染中被培養出來。只有一些病例報告由此菌造成的肺部或胸腔感染。我們報告一例在一個五歲大男孩因肺炎引起的右側膿胸中培養出侵蝕艾肯菌。我們做了胸管引流,但兩天後併發了胸部壞死性筋膜炎。這樣複雜的情況需要廣泛的手術,強力的抗生素及長期的住院照顧。由於這種病菌罕見,並且具有潛在的侵襲性,我們提出此報告提醒注意病菌。

Eikenella corrodens, a normal oropharyngeal flora, is a potential pathogen in human bites and head/neck infections. This organism is seldom cultured in parapneumonic infections in adult or pediatric populations. Only a few case reports mention pleuropulmonary infections caused by this pathogen. We report a 5-yearold boy with right-side parapneumonic effusion in which Eikenella corrodens was cultured. A closed-thorocostomy tube was used initially for drainage, and necrotizing fasciitis of the chest wall complicated this procedure 2 days later. This complex condition necessitated extensive surgery, strong antimicrobial therapy and prolonged hospitalization. Because of the rare occurrence and potential invasiveness of this disease, we present this case to emphasize the importance of awareness of this potential pathogen.
