Distal Clavicular Fracture Associated with Rupture of Coracoclavicular Ligament - Cases Report
外側鎖骨骨折合併喙鎖靭帶損傷個案報告
摘要
鎖骨骨折系人體骨折常見部位之一;此骨折一般雖可非手術法治療而得到良好的結果。然於外側鎖骨骨折伴隨喙鎖韌帶損傷時，若以非手術法治療，則因期有不癒合之高比率，往昔之學者( Neer )曾據此建議以手術整腹法來治療此類骨折。 另一方面，當外側鎖骨骨折伴有喙鎖韌帶剝離時，倘無適切地復位，將來導致鎖骨外型之改變，而影嚮及其於肩關節活動時所扮演的曲軸之角色;並進而增加肩峰一鎖骨關節所承受之張力，而使關節炎產生率增高。本文僅就生物力學之觀點探討正確之斷骨解剖復位對外側鎖骨骨折合併喙韌帶損傷之治療所具的意義，並提出病例以為報告。

Four patients are reported, one older and three young adults, each of them suatained fracture of the lateral part to the clavicle combined with coracoclavicular ligamentours ligamentous rupture. In view of biomechanic change in acromioclavicular joint motion following malaligned clavicle, we convince the functional recovery of injured shoulder should consist in accurate reduction of fractures. Failure to reduce the fracture by operation may lead to permanent deformity and affect the functional recovery.
