顯微手術的進步使得骨科的領域擴展了許多,但是顯微手術是個困難而且需要技巧的手術,因此在骨科醫師中精通此手術技巧並不普遍,最大的原因之一為缺乏一個良好的訓設計來幫忙醫師熟悉此技巧,我們設計一套實驗室內的動物實驗,採用漸進而完全原則,讓學習者利用手套縫合板先熟悉顯微鏡下操作,次採用老鼠練習微小血管末端對末端,末端對側端,及靜脈中開移植的吻合手術,以及微小坐骨神經吻合手術,皮瓣移值則練習腹股皮瓣移植。兎子是第二常使用之動物,由於兎耳之中央動脈及靜脈之大小與手指血管相近,所以經由兎耳的截斷及重接可作為臨床上面手指再接的練習模式。我們深信對顯微手術的認識與精通必須經由實驗室實際上動物的手術來學習,此套顯微實驗室所採用,受此訓練後之骨科醫師均可進行肢體或指頭再接之手術,更加證實此套設計的可行性。
The advance in microsurgery opened new field in orthopaedic surgery, yet the lack of good teaching manual hindered the popularity of microsurgery among orthopaedists. A systematic comprehensive design of basic laboratory microsurgical skills was proposed for the orthopaedists to follow. Most of the clinical works of orthopaedic microsurgery could be mastered through practicing the similar procedures on the animal models of this program. Rats and rabbits were the only two animals used because they were easily raised and handled in addition to the advantage of low cost. The results of experiments by the author was also presented for rthe reference of the reader. A step by step practice of this program was strongly recommended. This program is currently advocated in the microsurgical laboratory of NTUH and it is prved to be an effective teaching guide in microsurgery.
