Multiple Noncontiguous Vertebral Injury -A Case Report and Review of the Literature

脊椎多處損傷—病例報告

摘要


脊椎多處損傷雖然並不常見,但在其臨床上卻是十分重要的這類病例往往由於第二處或第三處損傷未及時發現而導致神經傷害擴大,脊椎不穩定,甚至可導致日後之畸型。 本院最近有一脊椎受傷病例,雖然立即將其整個脊柱以常規放射線照像術詳細的檢查,但是仍然延誤診斷第二處病灶達三十一天之久,而導致其神經傷害後遺症。作者在其參考文獻中尚未發現有類似此病例之病例報告,知道這類病例的存在,並在評估遭到相當程度之脊椎傷害之病患時,除以常規放射線照像術檢查外,並輔以脊髓顯影術檢查,對於早期診斷類似此病例的第二處或第三處脊椎損傷必有相當大助益,或可防止其併發症之發生。

Multiple noncontiguous vertebral injuries although are uncommonly see in orthopedic practice but are clinically important. Often the second or the third levels of injury are not recognized early enough to prevent clinical significant extension of the neurologic deficit, spinal instability and/or deformity. Recently, the author encountered one spinal injury case. Although the entire spine was evaluate carefully by standard radiography, he still missed the secondary lesion, a massive hereniation of intervertebral disc which was induced by trauma and cause cauda equine syndrome. On account of the delay in diagnosis and treatment, the patient was left with serious and permanent neurologic residuals. No similar cases were found in a review of the literature. Knowledge of this case and early myelography to augment standard radiography for assessment of patient sustaining a significant degree of spinal injury may be of significant aid early recognition of the similar secondary or tertiary lesions like this case presented and possible prevention of their complications.
