Closed Lateral Retinacular Release for Patellar Malalignment

摘要

自1984迄1987年，總共有46例髕骨排列不良的患膝接受閉鎖式髕骨外側韌帶放鬆術治療。今年九月間27位病人（43患膝）回到台北市立中興醫院接受追蹤評估。其中，男性4人，女性23人。平均年齡是36.3歲。在27人中6人左膝，5人右膝及16人兩膝接受治療。平均追蹤期是10.9月。主觀評估30.2%患膝術後病人感到滿意。
以Lysholm方式客觀評估，則有34.9%列「極優」級，41.9%列「良好」級，16.3%列「尚可」級，以及7.0%列「不良」級。以「極優」和「良好」合計為「滿意」級，則滿意率達76.8%。術前x光顯示95.3%患膝有髕骨外移，但追蹤評估時則僅8.7%外移如故。本系列除3例合併關節內出血外，無其他併發症。總結，髕骨外側韌帶放鬆術對於髕骨外移症候群不失為簡單有效之療式。

From 1984 to 1987 46 knees with patellar malalignment were treated with closed lateral retinacular release in Municipal Chung-Hsin Hospital of Taipei. In September 1987 27 patients (43 knees) returned for follow-up. There were 23 females and 4 males. The mean age was 36.3 years old. Among 43 knees operated 6 were left, 5 right, and 16 bilateral. The mean perind of follow-up was 10.9 months (range, 2 to 39 months). Subjectively 13 of 43 knees (30.2%) were satisfied with the operation. Using Lysholm's scaling 15 of 43 knees (34.9%) were rated ”excellent”, 15 (41.9%) ”good”, 7 (16.3%) ”fair”, and 3 (7.0%)”poor”. The satisfactory rate (”excellent” plus ”good” rate) was 76.8%. Preoperatively in 41 of 43 knees (95.3%) the patella showed lateral pull (or tilt) on Hughston's view, and at follow-up only 2 (8.7%) remained malaligned in the same degree. There were 3 knees (7.0%) complicated with hemarthrosis. Lateral retinacular release was concluded a simple and effective procedure in combating with patellar malalignment.
