Uncemented Ceramic Total Hip Arthroplasty -2 to 7 Years Follow-up

摘要

自一九七九年十二月至一九八四年十二月共有一四七個病人，一八0個髖關節在本院施行陶瓷人工髖關節置換手術。平均追蹤檢查時間為三、四年（二年至七年）。平均手術年齡為四三、六歲。病因以股骨頭壞死最多，佔62%，其次為退化性關節炎，佔22%。根據Mer d' Aubigne and Poste臨床評估結果有89%病人得到滿意的結果，但是根據本院新的評估方法（功能評估及x光評估）則只有47得到滿意的結果。使用此種人工髖關節所面臨最大的問題是相當高的鬆動率47.4%（臼杯部14.9%，莖幹部9.1%，兩者都鬆動者23.4%），杯狀物向上移位（44.8%），手術後大腿部痛（55%），但是其再手術率很低（1.7%）。本文認為使用此種人工關節仍需再長時間評估才能確定其好壞。

There were 180 hips (147 patients) undergoing cementless Autophor ceramic total hip arthroplasty between December 1979 and December 1984. The average period of follow-up was 3.4 years (2 to 7 years) and the average age of patients at operation was 43.6 years. The most cause of disease was osteonecrosis (62%) followed by osteoarthritis (22%). They achieved 89% good to excellent result according to the d'Aubigne and Postel score system. While using the newly developed score system (function + radiology), they can only achieve 47% good to excellent result. The major problems in using this type of prosthesis are high loosening rate (47.4%: cup 14.9%, stem 9.1%,cup and stem 23.4%), high cup proximal migration rate (44.8%) and postoperative thigh soreness (55%) in our follow-up. However, the revision rate is rather low (1.7%)
