摘要

四肢長骨骨折併發動脈受傷是骨科急診，並不是罕見的傷害。台大院在過去7年之中經歷2051例長骨骨折，其中12例併發動脈受傷，3例接受骨外固定術治療，8例行內固定術，一位接受石膏治療;斷裂血管直接縫合有6例，應用隱靜脈移植有3例，用人工血管有3例，我們結困顯示有6例得到良好結果。3例例發生關節戀縮現象以及3例接受截肢手術。迅速的重建血管，良好的骨折固定以及徹底的清創手術是處理四肢骨折併發動脈受傷之原則。

Fractures and dislocations are not uncommonly complicated by arterial injuries. The limb may not survive if vascular reconstruction is not performed rapidly.
From Jan. 1984 to May 1990, twelve patients suffered from vascular injuries associated with long bone fractures were treated at National Taiwan University Hospital. We studied the methods of treatment in managing long bone fractures and vascular injuries. The possible time limit of attempted peripheral vascular reconstruction was also investigated. The outcome of the patients was related to the methods of vascular repair and the time elapsed from injury to repair. However, there is no absolute time limit for attempted peripheral vascular reconstruction. Eleven patients received vascular reconnection within 24 hours after injury and there was no difference in end-result between patients operated within 8 hours and patients operated beyond 8 hours. Prompt re-establishment of peripheral circulation, adequate debridement and prevention of infection were the key points in managing vascular injury with long bone fractures.
