髖臼部金屬外臼與塑膠內臼早期脫節(病例報告及文獻回顧)

摘要


近幾年髖部全人工關節發展神速、而其發展趨向於多功能式組合。目前較為普及化的髖臼全人工關節則由金屬髖部外臼以及高硬度塑膠內臼所組成。此種可分離式髖臼I雖然便於臨床使用,但實際亦增加髖臼部內臼與外臼脫節可能性。 本篇報告是討論一位病患,曾經接受髖關節全人工關節置換術及發生脫臼現象。經徒手復位術後,X光片顯示異常因而接受手術復位術。手術中發現人髖臼的塑膠內臼與金屬外臼顯示脫離現象。因此面對髖臼內人工關節脫位時亦要考慮內臼與外臼脫離的可能性。

A case of early failure of a total hip arthroplaty presenting as a dislocation is presented. After attempts to reduce closely failed, lene insert of the acetabulum had separated from its outer metal backing. In recent years, the introduction of cementless prostheses and the rapid development of new designs have resulted in a variety of selection of the prosthesis during total hip arthroplsty. Currently used prosthesis designs consist of various kinds ofmaterials and methods. Several manufacturers use a metalbacked plastic cup to replaced the acetabulum, and in many of these, the polyethylene insert is snapped inot place after the metal backing has been fixed to the pelvis.
