Long-Segment Spinal Extradural Angiolipoma: A Case Report and Literature Review
多節性胸椎硬脊膜外血管性脂肪瘤－病例報告及文獻回顧
摘要
硬脊膜外血管性脂肪瘤是一種少見的良性腫瘤，組織學上包含了不同比例的血管和脂肪組織。文獻上所報告過的病例均不超過8節，本例共有11節。病人是位25歲的年輕女性，主訴有兩年多的右下肢無力，雖曾接受過檢查，郤沒有確定的診斷。住院後以核磁共振掃描，顯示從第二胸椎至第十二胸椎硬脊膜外後方有壓迫性腫塊。經由手術切除腫瘤，病理組織切片檢查證實為胸椎硬脊膜外血管性脂肪瘤。由於本病為良性，在神經學上的變化為壓迫所造成，只要能早期診斷及早期手術，神經病變均可以得到完全復原。

A case of extradural angiolipoma with spinal cord compression is reported.A 25 year-old female debeloped gradually progressive lower limbs weakness during a two-year period. The plain radiograph of the spine demonstrated mild scoliosis. Magnetic resonance imaging (MRI) showed a picture of high signal intensity in both T1 and T2 weighted images in the posterior extradural space from T2 to T12. When laminectomy from T2 to T12 was done, an extradural hypervascular fatty mass, which was loosely adherent to the dura mater, was disclosed and removed. Angiolipoma was impressed by the pathological findings. The clinical, radiological, and pathological characteristics of spinal extradural angiolipoma are discussed.
