280 cementless total hip arthroplasties in 232 patients were studied retrospectively. The follow-up period was 3-11 years. Three types of hip prostheses were used: Mittelmeier ceramic, Mecron and Roy-Camille. Localized osteolysis occurred in 8/134 Mittelmeier, 3/41 Mecron and 4/105 Roy-Camille prostheses. The median follow-up period was 9 years for Mittelemeier, 7 years for Mecron and 5 years for the Roy –Camille. Other signs of loosening was found in 7 Mittelmeier and 1 Mecron and they were revised. Tissue samples from osteolytic areas were examined. The osteolysis was found to be due to a foreign body reaction. It appeared later with the ceramic prosthesis than with the other two designs ( p < 0.025). it was concluded that ceramic was less likely than polyethylene to induce osteolysis because of lower wear rate.
